This form is used for submissions request for change of UKT Category MHS FT ULM
ATTENTION :
1) The proposed change to the UKT category is only for students who have studied for at least 2 semesters.
2) Deadline for submitting proposals to change UKT categories:
a. Odd Semester: MAY of the current year
b. Even Semester: OCTOBER of the current year
3) Before filling out this form, make sure you have prepared:
a. File of proof of filling in the FT ULM Student questionnaire for the current semester period.
