The following is the FT Dean's Letter No. 2608/UN8.1.31/SP/2021 regarding Data Collection on Face-to-Face Learning Preparations for ULM FT as a follow-up to ULM Chancellor's Circular Letter No. 3 of 2021, Letter from the Dean of FT ULM No. 2475/UN8.1.31/SP/2021 and No. 2593/UN8.1.31/SP/2021.

For this reason, students from the classes of 2020 and 2021 who have not yet filled in the data collection form and all active FT ULM students (aside from the classes of 2020 and 2021), are required to fill in the FT ULM Face-to-Face Learning Preparation Data Collection form via:
https://forms.gle/tQ2XmKEHqjxoHu7p6 (can only be accessed using an email account @ulm.ac.id)
no later than October 20 2021 at 16.00 WITA.

Proof of filling out the data collection form will be used as a condition for academic administration services for FT ULM students and as a condition for students' permission to carry out activities in the FT ULM campus area.
