This form is used to collect data on ULM FT Internet Quota Assistance as a follow-up to ULM Vice Chancellor's Letter for General Affairs and Finance No. 2523/UN8/TU/2020 dated 06 August 2020.
To avoid failure to inject internet quota as happened in the previous assistance period, then:
1) Make sure the registered telephone number is valid, active and has credit available (not within the grace period)
2) Make sure the provider data from the registered telephone number is CORRECT
3) This telephone number data collection is only for providers: Telkomsel, XL, Indosat and TRI
4) Data collection will be carried out until August 22 2020. Outside this date, no services will be served.
