This service is used to submit Certificates:
1) Loss/damage of diploma/transcript
2) Correction in writing diplomas/transcripts

Before submitting an application through this service, please prepare:
1) Scan file (not photo) of a valid official identity card (KTP/SIM).
2) Scan file (not photo) Proof of Lost Report from the police or relevant Certificate as proof of Loss/damage of Diploma/Transcript
3) Scan file (not photo) of Birth Certificate, High School Diploma or other relevant certificate as proof of correction in writing the Diploma/Transcript
4) Please fill in according to the example so that there are no editorial errors in the printed letter, because the letter will be printed automatically based on the data you filled in on this service form
