This form is used to submit a Letter of Recommendation from the FT Dean for the following purposes:
1) Replacement of KTM (Student Identity Card),
2) Application for Academic Leave,
3) Application for Dispensation for Late Payment of UKT/SPP, and
4) Take your diploma early

1) Prepare a file of proof of filling in the FT ULM Student Questionnaire for the current semester period before filling in this Submission Form. If you haven't filled out the FT ULM Student Questionnaire, please fill it in via: https://ft.ulm.ac.id/id/tracer/ (select the "Student Questionnaire" menu) or via: https://ft.ulm.ac.id/id/kuesioner-mahasiswa/
2) Please fill in according to the example so that there are no editorial errors in the printed letter.
